This is why some men experience pain despite normal blood tests or ultrasound results. The source may actually be coming from elsewhere.
2️⃣ Lower Back Dysfunction
Most of the nerves that supply the testicles originate from the lower back (lumbar spine). If there is stiffness, poor movement control, prolonged sitting posture, or a history of back strain, it can influence how these nerves function.
Sometimes, what feels like a “testicle problem” may actually be related to how your lower back is handling load and movement.
3️⃣ Pelvic Floor Muscle Tension
Your pelvic floor muscles sit at the base of your pelvis and play an important role in bladder control, bowel function, and sexual function.
If these muscles are too tight, overactive, or unable to relax properly, they can create aching, heaviness, or sharp discomfort in the testicles and perineal area.
Many men don’t realise they may be unconsciously clenching these muscles — especially during stress.
Why Does It Hurt After Sitting?

Pressure around the pelvic region
Lower back mobility
Pelvic floor muscle tension
Sitting for long hours can compress sensitive structures and keep certain muscles in a shortened position, which may trigger discomfort over time.
Why Does It Hurt After Ejaculation?
During ejaculation, the pelvic floor and cremaster muscles contract rhythmically to help expel semen.
If these muscles are tight or unable to fully relax after contraction, it may lead to lingering discomfort or aching. This does not necessarily mean something is “damaged” — sometimes it simply means the muscles are overactive or fatigued.
